Guidelines for Enrollment

NEW GRADUATE STUDENTS
STEP

  1. PRESENT TO TABLE 1 (GUARD'S TABLE AT LARA HALL) THE FOLLOWING:
    • ACCEPTANCE LETTER FROM NGOHS
    • SAIS ACCOUNT
    • PLAN OF COURSE WORK/PROGRAM OF STUDY
    • MEDICAL CERTIFICATE FROM UP HEALTH SERVICE
    • STUDY PERMIT FROM OSA (FOREIGN STUDENTS)
  2. GET THE TUITION CALCULATION FORM AND ENROLLMENT CHECKLIST AT TABLE 1 AND PROCEED TO ASSIGNED ADVISER
  3. PROCEED TO COMPUTER LAB AT THE 2ND FLOOR FOR PRE-ENLISTMENT WITH THE HELP OF CPH STUDENT COUNCIL
  4. SUBMIT THE ENROLLMENT REQUIREMENTS TO THE CPH STUDENT COUNCIL FOR TUITION CALCULATION & PAY P50 FOR CPH STUDENT FUND
  5. WAIT FOR THE PROCESSING OF THE TUITION CALCULATION AT THE COMPUTER LAB
  6. ONCE THE TUITION CALCULATION IS DONE, PAY TUITION FEE AT THE CASHIER'S OFFICE/LANDBANK
  7. GET UPM ID AT THE OFFICE OF THE UNIVERSITY REGISTRAR (OUR) AT PADRE FAURA; IF YOU PAID AT THE BANK, SUBMIT THE DEPOSIT SLIP TO OUR
  8. SUBMIT A COPY OF THE OFFICIAL RECEIPT TO THE OCS OR ONLINE (amsalazar2@up.edu.ph)

OLD STUDENTS
(UNDERGRADUATES AND GRADUATE)
Step

  1. PRESENT TO TABLE 1 (GUARD'S TABLE AT LARA HALL) THE FOLLOWING:
    • PRINTED FORM OF ENLISTED SUBJECTS APPROVED BY THE ADVISER
    • MEDICAL CERTIFICATE FROM UP HEALTH SERVICE
    • PRINTED GRADES OF PREVIOUS SEMESTER FROM SAIS
  2. GET THE TUITION CALCULATION FORM AT TABLE 1 AND PROCEED TO COMPUTER LAB AT 2ND FLOOR (WAITING AREA) TO FILL UP THE FORM
  3. PAY A CORRESPONDING FEE OF P50 AS STUDENT FUND TO CPH STUDENT COUNCIL BOOTH AT THE COMPUTER LAB
  4. RETURN THE DULY ACCOMPLISHED FORM AND OTHER REQUIREMENTS TO CPH STUDENT COUNCIL AT THE COMPUTER LAB
  5. WAIT AT THE COMPUTER LAB UNTIL TUITION CALCULATION IS DONE
  6. ONCE THE TUITION CALCULATION IS DONE, PAY TUITION FEE AT THE CASHIER'S OFFICE/BANK; IF YOU PAID AT THE BANK, SUBMIT THE DEPOSIT SLIP TO OUR
  7. GET ID STICKER AT THE REGISTRAR'S OFFICE
  8. SUBMIT A COPY OF THE OFFICIAL RECEIPT TO THE OCS OR ONLINE (Graduate: amsalazar2@up.edu.ph/Undergraduate: jacervantes@post.upm.edu.ph)
